P
rovides individuals 60 years and older, the direct linkage to many vital services and benefits available in Hawai`i County.  Staff will assist older individuals and their caregivers by:
 

  • Provide Information and Assistance regarding services and benefits available throughout the Aging Network of the Big Island of Hawai`i.
     
  • Provide Referral services for individuals to appropriate community resources by:
    1. assessing the needs of the individual
    2. identifying appropriate resources
    3. providing information to the individual so they may make informed choices
    4. linking the individual to needed services
    5. following up on referrals made to assure needs are met
       
  • Increasing awareness of the opportunities available in the community;
     
  • Following-up to ensure that linkage between the older individual or their caregiver was made with the appropriate service;
     
  • Advocate on behalf of those who may need special support.
     
  • Provide Outreach in the community to increase awareness of services and benefits available and their value to the community.
     
  • Assisting with application forms
     
  • Issue the County of Hawai`i's Senior Citizen ID Card Schedule and the Senior Discount Booklet which offers discounts from various business establishments;
     
  • Maintaining an up-to-date resource file and Resource Directory, and assisting the Hawai`i County Office of Aging in maintaining essential information about older persons, and
     
  • Providing assistance with other key services, such as:

For individuals 60 years and over who, for a variety of factors such as illnesses or disabilities, are unable to use conventional means of transportation or lack the resources necessary to reach needed services. CSE staff will provide transportation services to the disabled under 60 years of age in the Hilo district and with limited services in other districts on a priority basis. Contributions are accepted and the suggest contribution is $2.00 per trip. Priorities will be given in the following order: medical care, access to resource agencies to quality for benefits/services, essential shopping assistance and others; and

Includes routine housecleaning, laundry, meal assistance/planning, marketing, shopping assistance, personal care, running errands and escort services for any disabled/older adult in need of assistance to achieve, restore, or maintain independent living and self-direction, Priority will be given to older individuals having physical and/or mental impairments which require one or several supportive services in order for them to carry out normal activities of daily living and to remain in their own homes as long as possible, rather than be institutionalized.

Includes:  friendly visiting, financial assistance, interpreting/translating, letter writing and reading, Case Management, Senior Citizen ID Card Schedule, telephone reassurance, housing assistance, assisting other agencies/organizations with food surplus distribution, state and federal income tax assistance, low income home energy assistance, meal delivery and being designated as a volunteer station for other programs such as RSVP, Senior Companion Program, Senior Training & Employment Program, Elderly Recreation Services, Special Programs, etc.
